Service Description

This option is for current SHAPE ReClaimed clients that have prepaid for appointments. Individuals that do not apply to this scheduling option will be taken off of the schedule. If you have not prepaid for your appointment, please choose the Telehealth Consultation.

Cancellation Policy

By scheduling and appointment you understand that payment is required in full prior to service or purchase unless arrangements were agreed to in advance. Cash, check, ACH transfer, and credit are the only accepted forms of payment at this time. Notice of 24 hours is necessary for cancelled appointments. You may be charged a $15 fee for a missed appointment if canceled in less than 24 hours of appointment time
